Quality Community Nursing for Veterans

We provide respectful, trauma‑informed nursing support so veterans can stay safe, well and independent at home. Our experienced community nurses work closely with you, your family and your DVA team to coordinate care, monitor health changes and respond early when extra support is needed.

What is DVA Community Nursing?

The DVA Community Nursing Program helps veterans and their families receive professional medical care in the comfort of their own homes. At Taraja Care Services, our qualified and professionally registered clinical nurses provide personalised support, from wound management to complex health monitoring, fully funded by the Department of Veterans' Affairs for eligible members. This program is designed to support your wellbeing and independence, ensuring you receive respectful care without the need for hospital visits.

Who can refer?
• The General Practitioner (GP)
• Treating doctor in hospital
• Hospital discharge planner

Once we receive your referral, we arrange an assessment, start your care and review your needs as they change.

  • Complex wound management and dressing changes

We accept DVA Gold, White and Orange Cards for nursing services. Once approved, there is no cost to you. White Cards must cover community nursing.
